Thrive Magazine: Your plan, your voice

This issue of Thrive Magazine, Your plan, your voice, was informed by the Young People’s Care Forum in Wales which presented workshops that explored the importance of having a pathway plan to help prepare for leaving care and detail the support that young people are entitled too. This issue of Thrive looks at the what, when and how of pathway planning and how young people can ensure that they are involved in the whole process.
