Adult Practice Reviews (APRs) highlight systemic strengths and weaknesses in safeguarding after incidents involving adults at risk. A 2025 NISB review of APRs (2016-2022) identified recurring issues in Wales: poor multi-agency coordination, limited advocacy, inconsistent consideration of mental capacity and variable documentation of adults’ wishes. Positive practices included consistent use of advocacy services, “This is Me” person-centred documentation, and innovative collaboration. In this presentation, Michelle McManus and Emma Ball share findings from the review, policy implications, and how learning from APRs should inform the SUSR process, offering a call to action on embedding systemic, person-centred, and accountable approaches to adult safeguarding.
